Job Summary
Our client is seeking a skilled Care Advocate responsible for engaging with members to provide exceptional customer service, manage appointments, and ensure access to care. You will be integral in delivering personalized recommendations and collaborating with cross-functional teams to optimize the member experience.
Key Responsibilities
- Prioritize tasks and manage a schedule to conduct introductory video appointments with members.
- Assist members by answering questions about our product and providing personalized recommendations.
- Navigate member networks to ensure access to care and investigate available support.
- Coordinate with team members to provide immediate access to care and follow-up.
- Provide educational resources related to program needs and deliver necessary materials.
- Collaborate with cross-functional departments to enhance care delivery and customer support.
- Identify referral needs and collaborate with providers.
- Address members' social determinants of health needs.
Qualifications
- A background in allied health, human services, or healthcare.
- 1-2+ years of direct patient care experience.
- Strong problem-solving skills and ability to work in a remote/startup environment.
- Effective communication skills, both written and verbal, and a passion for positive member experiences.
- Experience achieving key performance indicators related to engagement and productivity.
- Proficiency in navigating new technologies and systems remotely.
- Certification or licensure in a healthcare-related field is preferred.
Skills
- Experience in family planning and maternal-child health areas is preferred.
- Comfortable with technology tools like Google Suite, CRM platforms, EHRs, and Zendesk.
