Job Summary

Our client is seeking a dedicated Personal Care Attendant responsible for providing daily living support to participants. The role includes assisting with Activities of Daily Living (ADL) and Independent Activities of Daily Living (IADL), ensuring a safe and clean environment, and accurately documenting services in electronic health records.

Responsibilities / Job Duties

• Provide Activities of Daily Living (ADL) and Independent Activities of Daily Living (IADL).
• Complete safe transfers using Durable Medical Equipment (DME) such as lifts, gait belts, and hoyers.
• Maintain cleanliness of participant's bathroom and home environment.
• Provide appointment reminders and ensure orderly conditions in centers or homes.
• Document participant services accurately and on time.
• Act as a participant advocate by reporting any violations of the Participant Bill of Rights & Responsibilities.
• Respond to participant complaints and report changes in their condition.
• Ensure a courteous, helpful, and professional attitude.
• Provide a safe environment by displaying awareness of safety hazards.
• Participate in the Quality Improvement program as required.
• Follow all OSHA safety guidelines and maintain confidentiality of participant information.
• Assist with individual and group activities, accompany participants to medical appointments, and ensure they wear code alert devices.
• Assist with food service programs and kitchen duties.

Minimum Education and Experience Qualification Requirements

Education

• High School Diploma or equivalent is preferred but not required.

Qualifications

• Minimum 1 year of experience working with frail, low-income elderly is preferred.
• Professional demeanor with patience for participants.
• Ability to communicate clearly and effectively with participants.
• Good written skills for daily activity charting.

Schedule / Shift

M-F 8am-5pm
