Job Summary
Our client is seeking a dedicated health coach to join their Care Delivery team, providing exemplary care services to members. As a Care Coach, you will coordinate care beyond the digital space, drive behavior change through a shared coaching model, and support members 24/7/365.
Key Responsibilities
- Engage and convey the benefits of the platform to optimize care program opt-in rates.
- Create SMART goals with members, ensuring adherence and refinement within clinical program design.
- Conduct appointments, establish relationships, and manage caseloads.
- Adhere to clinical program guidelines, utilizing motivational interviewing skills.
- Assess member knowledge, adapt education approaches, and collaborate with care teams.
- Monitor member health data and ensure seamless access to care.
- Embrace continuous learning to improve quality of care and clinical outcomes.
- Collaborate closely with cross-functional stakeholders and address social determinants of health needs.
Qualifications
- 1-2+ years of experience in health coaching.
- Ability to remain flexible with changes and possess a growth mindset.
- Strong communication skills and proficiency in new technologies.
Skills
- Experience achieving KPIs, including opt-in rates, follow-up adherence, and member satisfaction.
- Experience with technology tools like Google suite, CRM platforms, EHRs, and Zendesk.
Certifications
- Preferred certifications: ICF or NBC-HWC. Candidates preparing to sit for exams are welcome.
Additional Requirements
- Preferred experience in family planning, maternal-child health, and clinical practice in nursing or behavioral health.
- Experience in a start-up or fast-paced environment.
