Job Summary

Our client is seeking a dedicated health coach to join their Care Delivery team, providing exemplary care services to members. As a Care Coach, you will coordinate care beyond the digital space, drive behavior change through a shared coaching model, and support members 24/7/365.

Key Responsibilities

• Engage and convey the benefits of the platform to optimize care program opt-in rates.
• Create SMART goals with members, ensuring adherence and refinement within clinical program design.
• Conduct appointments, establish relationships, and manage caseloads.
• Adhere to clinical program guidelines, utilizing motivational interviewing skills.
• Assess member knowledge, adapt education approaches, and collaborate with care teams.
• Monitor member health data and ensure seamless access to care.
• Embrace continuous learning to improve quality of care and clinical outcomes.
• Collaborate closely with cross-functional stakeholders and address social determinants of health needs.

Qualifications

• 1-2+ years of experience in health coaching.
• Ability to remain flexible with changes and possess a growth mindset.
• Strong communication skills and proficiency in new technologies.

Skills

• Experience achieving KPIs, including opt-in rates, follow-up adherence, and member satisfaction.
• Experience with technology tools like Google suite, CRM platforms, EHRs, and Zendesk.

Certifications

• Preferred certifications: ICF or NBC-HWC. Candidates preparing to sit for exams are welcome.

Additional Requirements

• Preferred experience in family planning, maternal-child health, and clinical practice in nursing or behavioral health.
• Experience in a start-up or fast-paced environment.
