Location: Baltimore City, MD (Community-Based)
Position Type: Full-Time or Part-Time (Monday – Friday) Minimum of 20 hours per week
Pay Rate: $50.00 / hour (Billable Hours)

In this role, you will work directly in the community to conduct in-home assessments, develop individualized care plans, and monitor essential services for individuals receiving community-based care. Key Responsibilities
- Conduct comprehensive, in-home nursing assessments for CPAS clients within Baltimore City.
- Develop, evaluate, and update personalized plans of care to ensure client safety and wellbeing.
- Monitor community-based personal assistance services to ensure compliance and high standards of care.
- Collaborate with clients, families, and interdisciplinary care teams to advocate for client needs.
- Complete accurate and timely documentation for quality assurance and billing purposes.
