Job Summary
Our client is seeking a Physician to conduct comprehensive telehealth intakes, clinical assessments, and strategize patient care for Medicare beneficiaries. This role emphasizes clinical autonomy, enabling physicians to lead care management strategies without administrative burdens.
Key Responsibilities
- Conduct 30-60 minute clinical assessments for newly enrolled Medicare beneficiaries to establish the clinical foundation for ongoing care management.
- Review and synthesize complex medical histories to confirm eligibility for Chronic Care Management (CCM) services.
- Formulate tailored care strategies and direct non-clinical care managers and social workers for long-term coordination.
- Maintain clinical oversight into patient progress and conduct formal reassessments every 6 to 12 months.
- Document clinical findings using Canvas EHR system and ensure accurate time logging.
- Build rapport with elderly patients and their families in a telehealth setting while maintaining strict schedule adherence.
Qualifications
- MD or DO degree with active board certification in Family Medicine, Internal Medicine, Geriatrics, or related specialty.
- Possession of an active 50-state medical license or willingness to obtain additional state licensure.
- Telehealth proficiency with the ability to establish rapport and navigate discussions with aging adults.
- Technical setup capable of running Zoom and Canvas EHR.
Experience
- Substantial geriatric experience with chronic disease management and post-discharge care for older adults.
- Collaboration experience within a multidisciplinary care team.
Skills
- Proficiency with cloud-based EHR systems for clinical documentation.
- Excellent communication skills and ability to work in a remote setting.
