Job Summary
Our client is seeking an experienced Nurse Practitioner to work in a primary care setting with a focus on patients ages 55 and older. The role involves diagnosing and treating acute healthcare problems, providing routine care and assessments, and participating in developing comprehensive care plans. The nurse practitioner will also provide guidance and support to clinic staff.
Responsibilities / Job Duties
- Provide primary care for patients ages 55+ in a primary care setting.
- Diagnose and treat acute health care problems, including illnesses and injuries.
- Conduct routine assessments and order medications and devices as needed.
- Consult with staff physician as outlined in protocols.
- Analyze diagnostic information to develop appropriate diagnosis.
- Develop treatment plans and participate in comprehensive care planning.
- Counsel participants on drug regimens and health risk behaviors.
- Communicate care needs to external consultants and caregivers.
- Document care provided in participant medical records.
- Provide guidance to clinic nursing and support staff.
- Perform related duties as assigned.
Minimum Education and Experience Qualification Requirements
Education
- Masters Prepared Nurse Practitioner—Family, Adult, or Geriatric certificate.
- Licensed as a Registered Nurse in California and certified by the BRN.
Qualifications
- 1+ years of experience in primary care and/or geriatrics.
- DEA Certificate and Furnishing License required.
- CPR Certification.
Experience
- Experience and interest in working in geriatrics.
- Experience working with the frail or elderly is preferred.
- PACE experience preferred.
- Hospital experience preferred.
Schedule / Shift
Monday-Friday 8am-5pm (could extend if appointments run long). On call 1 day/month.
