Job Summary
Our client is seeking a motivated Community Health Worker to engage in field outreach and member engagement, assist in executing Social Determinants of Health (SDOH) solutions, and collaborate with interdisciplinary teams to support member care plans. This role involves door-to-door field visits within Washington, D.C., and assisting with various social barriers.
Key Responsibilities
- Conduct daily door-to-door field visits and neighborhood outreach, primarily focusing on Wards 7 and 8, to engage with assigned enrollees.
- Work collaboratively with clinical teams to verify member contact details and re-engage disengaged members in active case management.
- Execute interventions to address social barriers, including food access, housing resources, and basic needs.
- Cooperate with Nurse Case Managers and Licensed Clinical Social Workers to support person-centered care plans.
- Report daily field findings and ensure compliance with organizational protocols and confidentiality standards.
Qualifications
- Previous professional experience in field-based outreach, community navigation, or case management assistance with a Managed Care Organization (MCO).
- Must hold a valid driver’s license and maintain a reliable personal vehicle for navigation throughout D.C. zip codes.
- Strong knowledge of District of Columbia neighborhoods, particularly Wards 7 and 8.
- Proficiency in operating mobile tablets, smartphone applications, and safety tracking technology.
Additional Requirements
Position requires daily field deployment, with hours from 8:00 AM – 5:00 PM, Monday through Friday. The role involves up to 85% travel across D.C. and requires flexibility in working locations.
