Job Summary
Our client is seeking a Registered Nurse Case Manager who will be responsible for conducting, reviewing, and documenting comprehensive clinical and psychosocial assessments. The role includes facilitating care plans with the involvement of members and coordinating care across various healthcare services to ensure functional independence at home or in the community. The Registered Nurse Case Manager will also manage the transition of care for members moving between settings.
Responsibilities / Job Duties
- Conduct, review, and document comprehensive clinical and psychosocial assessments and on-going follow-up interventions.
- Maintain case files in accordance with policies and procedures, ensuring compliance with state and federal requirements.
- Facilitate involvement in development and implementation of care plans with the member and/or family.
- Coordinate member’s care with PCP, Specialists, Behavioral Health, and support providers.
- Facilitate successful transition of care for members between care settings.
- Support the transition of long-term care members to lower levels of care or community settings, if assigned to MLTSS.
- Conduct telephonic assessments.
Minimum Education and Experience Qualification Requirements
Qualifications
- Active California RN License without restriction.
- Minimum three years of experience in case management, discharge planning, or equivalent experience.
- Knowledge of managed care principles and practices with emphasis in Utilization Management and/or Case Management.
- Clinical knowledge and critical thinking skills necessary to develop an effective care plan.
- Proficient in operating computer applications such as Outlook, Word, Excel, and specific case management programs.
- Spanish, Vietnamese, Chinese, or Tagalog language bi-lingual skills preferred.
- Experience working with designated member population (e.g., behavioral health, seniors and persons with disabilities, children) preferred.
Schedule / Shift
Work onsite from 8:30 AM to 5:00 PM, Monday to Friday.
