Job Summary
Our client is seeking a Referral Coordinator responsible for staffing nationwide home health referrals for workers' compensation patients. This role involves coordinating care transition from hospitals to home settings, arranging various home health services, and managing communication channels to ensure effective care delivery.
Key Responsibilities
- Staff nationwide home health referrals for workers' compensation patients ranging from short-term to catastrophic cases.
- Coordinate care with hospitals/inpatient rehabs for patient transitions to home settings.
- Arrange home health services including nursing, PT, OT, speech therapy, and caregivers.
- Negotiate rates with home care providers to maintain margins within contract thresholds.
- Manage high-volume communication, including a call center phone queue and 100+ daily Outlook emails.
- Navigate a proprietary EMR system to update patient files and review medical records/prescriptions.
Qualifications
- Medical background/terminology – ability to review medical records and communicate with medical personnel.
- Proficient in Outlook email management and able to quickly learn new EMR systems.
- Exceptional multitasking skills in a fast-paced call center environment.
- Prior remote work experience.
- High level of professional communication and presentation skills.
- Strong attendance and punctuality due to dedicated case loads.
- Bilingual abilities in Spanish strongly preferred.
- Workers' Compensation background and experience with care coordination/home care agencies are advantageous.
