Job Summary
Our client is seeking a Licensed Practical Nurse who will play a key role in patient intake, clinical collaboration, patient experience, and providing hands-on clinical support. This position requires a focus on compliance and documentation while providing health coaching to senior patients.
Key Responsibilities
- Patient Intake & Screening: Conduct patient intake by measuring and recording vital signs, interviewing patients, and documenting thorough medical histories. Perform routine screenings while ensuring patients are well-informed and comfortable.
- Clinical Collaboration: Engage with patients throughout their appointments, immediately reporting any changes in health status or specific needs to the PCP or specialist teams.
- Patient Experience & Advocacy: Process patient phone messages, return calls, route clinical inquiries to appropriate team members, and relay critical physician instructions.
- Hands-on Clinical Support: Perform venipunctures, IV starts, dressing changes, bandage applications, suture removals, and other first-aid procedures under physician supervision.
- Health Coaching: Provide proactive wellness and health coaching to a defined group of senior patients, following up via scheduled weekly calls.
- Compliance & Documentation: Maintain accurate documentation within the EHR system, ensuring strict compliance with OSHA safety standards.
Qualifications
- Patient-Centered Approach: High degree of emotional intelligence, integrity, and a customer-oriented communication style when working with seniors and their families.
- Adaptability & Initiative: Self-starter with strong critical reasoning and problem-solving skills.
- Technical Savvy: Proficient with Microsoft Office Suite and comfortable using analytical clinical software packages.
- Collaboration: Ability to work effectively across various seniorities, including clinic staff, providers, market leadership, and vendors.
- Travel: Willingness to travel occasionally for corporate or regional team meetings.
Experience
- Experience working with the Geriatric Population.
- Experience working in Value Based Care Space.
