Job Summary
Our client is seeking a dedicated Family Practice Nurse Practitioner to leverage modern technology in providing telehealth services. The primary responsibilities include conducting virtual visits, evaluating health risks, and managing patient care coordination.
Key Responsibilities
- Review daily caseloads and prep technology for patient services.
- Use technology to manage schedules and coordinate with in-home clinical team members.
- Build connections with a diverse range of patients, gather health history, and prepare for virtual services.
- Conduct health risk assessments, medication reviews, and cognitive screenings.
- Collaborate with on-site team members to evaluate vitals and execute patient-tailored tasks.
- Manage preventive care, handle referrals, oversee medication management, and document clinical findings.
Qualifications
- Board Certified as a Family Nurse Practitioner.
- Active Family Nurse Practitioner License in California.
- California DEA license.
- Credentialed Medicaid provider for the state of California (Medi-Cal).
- 1.5+ years of clinical experience in family practice, primary care, or internal medicine.
- Several years of telehealth experience preferred.
- Bilingual (Spanish-speaking) capabilities are highly valued.
