Job Summary
Our client is seeking a dedicated Case Manager to join their team. As a Case Manager, you will play a crucial role in managing patient admissions and developing effective aftercare plans. Your primary responsibilities will include interviewing patients for case management, facilitating mental health groups, and maintaining communication with patients and their families.
Key Responsibilities
- Conduct case management interviews upon patient admission.
- Provide necessary documentation for employers, schools, and legal purposes.
- Meet with patients weekly for case management appointments.
- Develop and maintain strong relationships with patients and their families.
- Create and update aftercare plans, ensuring patient adherence.
- Collaborate with teams to determine patient discharge dates.
- Participate in team meetings to share information accurately.
- Ensure medical records are sent to appropriate facilities post-discharge.
- Facilitate Substance Use Disorder (SUD) and Mental Health groups.
- Understand health insurance benefits and policies.
- Reiterate program rules to patients and report any infractions.
- Respectfully interact with patients while maintaining professional boundaries.
- Protect patient information in line with privacy policies and laws.
Schedule
- Monday-Friday 8:00am-5:00pm
Qualifications
- 2+ years of experience in the behavioral health field.
- Associates level education preferred.
- Preferred but not required: LADC or CRSW/CADC certifications.
Experience
- 2+ years of experience working in the behavioral health field.
Skills
- Experience in behavioral health and case management.
- Strong communication and organizational skills.
