Job Summary
We are seeking a detail-oriented and proactive Referral Coordinator to facilitate, process, and track patient referrals. Operating within the Electronic Health Record (EHR) system during standard clinical hours, you will serve as the critical link between healthcare providers, insurance networks, community specialists, and patients. Your primary goal is to ensure patients receive seamless, timely access to specialty care while maintaining administrative accuracy and regulatory compliance.
Key Responsibilities
- Process Requests: Monitor and prioritize incoming referral requests within the EHR, managing high-volume workflows from multiple providers.
- Clinical Data Review: Audit referrals for completeness, navigating patient charts to interpret diagnoses, clinical orders, lab results, and CPT/ICD-10 codes.
- Administrative Accuracy: Evaluate referrals against standard insurance guidelines to catch and resolve errors early; collaborate directly with care teams to secure any missing clinical documentation.
- Insurance & Network Navigation: Maintain an in-depth understanding of insurance plans, covered benefits, and in-network community specialists.
- Stakeholder Liaison: Serve as a key point of contact between clinical teams and insurance companies for complex or high-risk patient referrals.
- Patient Advocacy: Coordinate with patients to assist them in scheduling specialty appointments, making outbound phone calls to guide them through the process when necessary.
- Referral Tracking: Utilize internal reporting tools to monitor referral statuses, identify outstanding or delayed referrals, and report critical updates back to clinical teams.
- EHR Documentation: Maintain meticulous, real-time documentation within patient charts to ensure a clear audit trail of the referral lifecycle.
- Inbound Communications: Manage incoming phone inquiries directed to the Referral Department, providing professional and helpful assistance.
- Network Maintenance: Verify the accuracy of external specialist contact directories (names, addresses, phone, and fax numbers) and report discrepancies to leadership.
Qualifications
- Referral experience
- Medical Knowledge: Strong command of medical terminology.
- Communication: Exceptional oral and written communication skills, with the ability to clearly explain medical and behavioral health concepts to community specialists.
- Technical Proficiency: Solid experience navigating web-based portals alongside standard Microsoft Office applications (Word, Excel, and Outlook).
- Collaborative Workstyle: Proven ability to work both independently and collaboratively as a cohesive member of a healthcare team.
- Relationship Management: Ability to build and maintain positive professional relationships with community specialists and insurance providers.
- Quality Focus: Strong attention to detail and organizational skills.
