Job Summary
Our client is seeking a dedicated Licensed Social Worker to partner with primary care teams in conducting comprehensive social services assessments. The goal is to identify patient needs and coordinate tailored support plans to help overcome barriers to medical care.
Key Responsibilities
- Partner with primary care teams to conduct comprehensive social services assessments and identify individual patient needs.
- Design and implement tailored support plans to overcome socioeconomic and logistical barriers to medical care.
- Assist patients and families in navigating managed care plans, community resources, and benefit programs.
- Provide ongoing behavioral and social case management.
- Support members through advance care planning and health transitions.
- Maintain accurate clinical documentation in electronic health record systems and participate in quality improvement initiatives.
Qualifications
- Bachelor’s degree in Social Work (BSW) required; Master’s degree in Social Work (MSW) strongly preferred.
- Active, unencumbered social work license (BSW or MSW level) in the state of practice.
- 1+ years of experience in clinical social work, with expertise in screening, assessment, and planning for complex patient populations.
- Practical understanding of differential diagnosis for common mental health conditions.
- Exceptional active listening skills and strong interpersonal communication.
- Proficiency with clinical management software and general computer systems.
- Highly empathetic, non-judgmental approach with a flexible, problem-solving mindset.
- Medical social work background or healthcare case management experience (preferred).
- Master of Social Work (MSW) or LCSW (preferred).
Hours Per Week
40.0 hours
Additional Requirements
Must be willing to visit all sites in the market, including traveling to Lincoln one day a week (1.5 hours route).
