Job Summary
Our client is seeking a dedicated Registered Nurse Case Manager to join their team. The primary responsibility of this role is to oversee and manage patient care plans, ensuring the delivery of high-quality, patient-centered care. The successful candidate will conduct in-home clinical assessments, develop individual patient care plans, and coordinate care with interdisciplinary teams.
Key Responsibilities
- Complete in-home clinical assessments to evaluate patient needs and support a smooth start to services.
- Develop, implement, and regularly update individualized care plans based on each patient's clinical needs and goals.
- Build strong relationships with patients and caregivers through ongoing home visits and routine clinical follow-up.
- Monitor patient health, identify changes in condition, address safety concerns, and remove barriers to quality care.
- Coordinate care with physicians, caregivers, internal clinical teams, and community-based providers to ensure continuity of care.
- Educate patients and caregivers on care plans, disease management, safety practices, and available community resources.
- Review caregiver documentation and provide clinical guidance when changes in a patient's condition arise.
- Advocate for patients by promoting high-quality, patient-centered care and ensuring their needs are met.
- Maintain accurate, timely clinical documentation in compliance with regulatory guidelines and organizational standards.
- Collaborate with an interdisciplinary care team to improve patient outcomes and enhance the overall patient experience.
Qualifications
- Graduated from an accredited nursing program.
- Active and unrestricted Registered Nurse license in the State of Colorado.
- Reliable vehicle + auto insurance.
- Experience with Medicaid populations.
- Experience within homecare is highly preferred.

Additional Requirements
- 3 days in the field and 2 days at home for admin/telehomecare evaluations.
