Job Summary
Our client is looking for a dedicated Team Lead to manage a small caseload of ACT clients and provide comprehensive support and supervision to staff and clients. The role involves connecting clients to necessary resources, conducting field visits, and participating in program operations and compliance.
Key Responsibilities
- Maintain a small caseload of ACT clients (4-5).
- Complete comprehensive client assessments and develop individualized treatment goals.
- Support clients in accessing benefits, psychiatric services, medication management, and rehabilitation programs.
- Connect clients to community healthcare, substance use treatment, and other supportive resources.
- Accompany clients to appointments as needed.
- Maintain consistent communication with clients’ families regarding needs, progress, and treatment planning.
- Serve as a liaison between clients and external service providers to ensure coordinated care.
- Maintain accurate and timely client documentation in accordance with program requirements.
- Conduct community-based visits and advocate for clients' needs.
- Participate in the rotating on-call schedule outside of regular business hours.
- Provide clinical supervision and support to staff and interns.
- Assist with enrolling eligible clients into the ACT program and support recruitment efforts for ACT team staff.
- Assist with program operations, staff scheduling, and ensuring completion of training requirements.
- Support compliance with OMH and ACT standards.
- Perform Program Director responsibilities in their absence.
- Participate in program billing processes and attend staff meetings, case conferences, and training opportunities.
- Collaborate with ACT team members using a multidisciplinary, team-based approach.
- Assist with the development and implementation of policies and procedures.
- Perform additional responsibilities and duties as assigned.
Qualifications
- Licensed Master Social Worker.
- Experience working with individuals diagnosed with serious and persistent mental illness required.
- Ability to walk and climb stairs as needed.
- Experience supporting young adult populations preferred.
- Prior ACT Team experience preferred.
- Bilingual or multilingual abilities (Spanish, French, Creole, American Sign Language, or other languages) are a plus.
- SIFI (Supervisor in Field Instruction) certification strongly preferred.
