Schedule: Monday – Friday, 8:00 AM – 5:00 PM (1-hour lunch)
Compensation: $52.00 – $58.00 / hour (Dependent on experience)
About the Role
Are you an experienced clinical social worker looking for a high-impact, rewarding short-term project? We are seeking a dedicated Temporary Care Coordination Manager (LCSW) to step in and champion the health, dignity, and independence of our Medicare Dual Eligible Special Needs Plan (D-SNP) members.
This role offers the comfort of a primarily remote work environment, but requires candidates to reside in California and live within reasonable driving distance of one of our regional hubs for occasional, very rare in-office needs. Under limited supervision, you will hit the ground running—actively managing individualized care plans, serving as a strategic guide through the managed care ecosystem, and collaborating with an interdisciplinary team.
What You'll Do (Responsibilities)

Design Individualized Pathways: Develop, implement, and manage member-centered intervention plans for D-SNP members referred to the Care Coordination program, driving optimal outcomes in a cost-effective manner.

Be the Bridge & Navigator: Work hand-in-hand with our interdisciplinary Care Coordination team to educate and empower members, caregivers, providers, and agencies as they navigate the managed care continuum.

Champion Program Quality: Participate directly in program evaluations and quality improvement initiatives, ensuring our D-SNP program remains highly effective and fully compliant with regulatory requirements.
What You Bring (Requirements)

Location & Residence: Must currently reside in California. Applicants must live within reasonable driving distance to at least one of the following locations for rare, occasional onsite commitments:

Licensure: Active and continuous maintenance, in good standing, of a Licensed Clinical Social Worker (LCSW) license issued by the State of California.

Education: A Master’s Degree in Social Work (MSW) or a closely related field.

Experience: At least two (2) years of social service experience within an acute care, community-based, or managed care environment. This must include direct experience developing and managing member-centered care plans.
Preferred Experience:

Prior experience navigating Medicare, Medi-Cal, and/or D-SNP populations.

Familiarity with Enhanced Care Management (ECM), Community Supports (CS), and/or Long-Term Services and Supports (LTSS) coordination.

Bilingual skills (English/Spanish or English/Hmong).
Assignment Details & Compensation

Job Type: Temporary hourly contract assignment.

Pay: $52.00 to $58.00 per hour, paid weekly based on experience and clinical expertise.

Predictable Schedule: Maintain an excellent routine with a structured, daytime Monday through Friday, 8:00 AM to 5:00 PM schedule.

Lunch: Includes a dedicated, unpaid 1-hour lunch break every day.
