Job Summary
Our client is seeking a qualified Application Developer responsible for providing primary care to a patient panel managing chronic kidney disease, multi-system comorbidities, and end-stage renal conditions. The role requires completing 20 visits per week, with 80% being in-home visits. You will also manage note closures and hospital transition visits to control medical expenses.
Key Responsibilities
- Provide primary care to a patient panel managing chronic kidney disease and multi-system comorbidities.
- Complete 5 visits per day / 20 visits per week.
- Ensure 80% of visits are in-home; the remaining 20% can be telehealth/virtual.
- Manage turnaround metrics for note closures and hospital transition visits.
- Maintain a maximum cap of 200 engaged patients per Advanced Practice Provider (APP).
Qualifications
- Minimum of 3 to 5 years of experience as a practicing APP.
- Core Primary Care background.
- No new graduates.
- Experience within a Value-Based Care model or home health visits preferred.
Additional Requirements
Monday through Friday, 8:00 AM – 5:00 PM schedule with a 1-hour lunch break. No flexibility for alternative shifts or 4/10 schedules.
