Job Summary
Our client is seeking a compassionate and detail-oriented Care Coordinator to support individuals in navigating their care journey. This role involves partnering closely with interdisciplinary teams to deliver short-term, goal-oriented interventions and ensure patients receive timely, effective support within a collaborative healthcare setting.
Key Responsibilities
- Provide brief, evidence-based interventions tailored to individual needs
- Collaborate with medical providers and behavioral health specialists to support care plans
- Monitor patient progress and adjust support strategies as needed
- Educate patients on treatment plans, medications, and available resources
- Track outcomes, document interactions, and maintain accurate records
- Identify barriers to care and proactively connect individuals to appropriate services
- Participate in team meetings and contribute to continuous care improvement efforts
Qualifications
- Strong understanding of care coordination, case management, or community-based support services
- Excellent communication, organizational, and documentation skills
- Ability to assess client needs, develop care plans, and coordinate services effectively
- Ability to manage multiple priorities in a fast-paced, team-oriented environment
- Commitment to supporting underserved and vulnerable populations
- Bilingual or multilingual abilities (Preferred)
- Experience working within Health Home, managed care, or integrated care models (Preferred)
- Familiarity with social determinants of health and community resource navigation (housing, benefits, food access, etc.) (Preferred)
- Experience supporting transitional care (hospital to community) (Preferred)
- Knowledge of local service systems and referral networks (Preferred)
- Proficiency with electronic health records (EHR) or care management platforms (Preferred)
- Background in behavioral health, substance use, or chronic disease management (Preferred)
Experience
- Minimum of 1 year of experience providing direct services to individuals with complex needs (e.g., serious mental illness, chronic conditions, or disabilities)
Education
- Bachelor’s degree in Social Work, Human Services, Psychology, Public Health, or a related field
Additional Requirements
- Hours Per Week: 40 hours per week
- Comfortable conducting field-based work, including home visits, hospital visits, and community outreach
- Reliable transportation and willingness to travel regularly, including transporting clients when needed
