Job Summary

Our client is seeking a Hospice Registered Nurse Case Manager to provide professional nursing care to patients in their homes. This role involves assessing, planning, implementing, and evaluating individualized care needs on an ongoing basis, ensuring all care is delivered in accordance with organizational policies, procedures, and applicable regulatory requirements.

Key Responsibilities

• Ensure timely execution of physician orders and maintain accurate documentation reflecting compliance with orders and patient progress.
• Deliver patient care in alignment with established policies, providing holistic care to patients and their families.
• Apply sound clinical judgment and critical thinking to support safe and effective patient outcomes.
• Identify appropriate payer sources and ensure compliance with authorization requirements for services.
• Complete all required documentation, including assessments, care plans, and interventions within timelines.
• Implement targeted patient education strategies aligned with care plan goals.
• Communicate patient care concerns promptly and ensure proper documentation.
• Serve as a liaison among physicians, clinical leadership, and healthcare team members.
• Communicate essential patient information and Plan of Care during care transitions.
• Supervise home health aide staff and conduct required supervisory visits.
• Stay current with evolving clinical standards and integrate best practices into patient care.
• Perform additional duties and special projects as assigned.
• Adhere to organizational competencies and meet physical requirements.

Qualifications

• Active New Jersey Registered Nurse License
• Basic Life Support (BLS) for Healthcare Providers (AHA certification required)
• Basic computer proficiency

Experience

• Minimum of six (6) months RN experience
• A minimum of 1 year in a med-surg or acute care environment is preferred

Skills

• Technical skills required: Basic computer proficiency

Shift

• Standard shift: 8:30 AM – 5:00 PM (Monday-Friday)
• Weekend requirement:
  • Every 4th weekend (Saturday & Sunday)
• Time off compensation:
  • 1 day off week prior
  • 1 day off week after
  • Scheduling decided by employer (flexible but business-driven)
