Job Summary
Our client is looking for a dedicated Quality Coordinator to execute local and enterprise strategies to close HEDIS and STARS care gaps. This role involves patient outreach, engagement, and collaboration to improve healthcare quality and compliance.
Key Responsibilities
- Quality Gap Closure: Execute local and enterprise strategies to close HEDIS and STARS care gaps.
- Patient Outreach & Engagement: Conduct outbound calls to patients to encourage, educate, and assist them with scheduling and completing preventative screenings.
- Clinical Records Management: Review medical records to document and close focus care measures; request external medical records and accurately upload documentation into the EMR system.
- In-Center Support: Schedule and perform ancillary visits to complete in-center screenings.
- Stakeholder Collaboration: Meet regularly with Market Leaders to discuss roadblocks, share progress reports, and strategize localized quality initiatives.
- Compliance & Safety: Ensure strict compliance with OSHA safety standards and maintain accurate, legal, and ethical clinical documentation at all times.
Qualifications
- Active working knowledge of healthcare quality measures and gap closure strategies.
- Proven experience working effectively within a primary care environment.
- Strong team-building, interpersonal, and collaboration skills.
- Ability to work independently with a high level of attention to detail and organizational focus.
- Proficient with Microsoft Office Suite (Word, Excel, PowerPoint, Outlook, and Teams).
- Sensitivity to the unique needs of culturally diverse staff, patients, and communities.
- Strict adherence to HIPAA standards and the confidentiality of protected health information (PHI).
- Preferred: Referral Experience and eClinical Works experience.

Additional Requirements
Monday-Friday, 8-5 schedule.
