Job Summary
Our client is seeking a Bilingual Coordinator to support individuals with medical, behavioral health, or social needs. The role involves providing person-centered care coordination to help members access services and navigate the healthcare system, thus improving health outcomes.
Responsibilities / Job Duties
- Conduct outreach to engage eligible members in care management services.
- Complete assessments to identify medical, behavioral health, and social needs.
- Develop and implement person-centered care plans with members.
- Coordinate services across healthcare providers, health plans, and community partners.
- Support members with appointment scheduling, transportation, benefits applications, and access to community resources.
- Provide education, advocacy, and ongoing follow-up to help members achieve their wellness goals.
- All tasks to be done virtually, supporting the team who visits these members in person.
- Maintain timely and accurate documentation and meet program engagement standards.
Minimum Education and Experience Qualification Requirements
Education
- High school diploma and equivalent work or lived experience serving similar populations.
Qualifications
- Minimum 1–2 years of experience in case management, community outreach, social services, behavioral health support, or similar member-facing work.
- Experience working with individuals experiencing homelessness, medical complexity, behavioral health needs, or social barriers.
- Must be bilingual in Spanish.
Experience
- Experience in care management, case management, social services, behavioral health, or community health with strong communication, organization, and relationship-building skills.
- Experience with Medicare/DSNP, Medi-Cal, or safety-net healthcare environments.
Schedule / Shift
Standard Business Hours - Monday through Friday 8:30am-5pm
