Job Summary
Our client is seeking a Primary Care Nurse Practitioner to join their collaborative and supportive healthcare team. The primary responsibilities include assessing individuals’ health status, providing patient care, evaluating nursing interventions, and collaborating with primary care providers and team members. Additional responsibilities include admin duties and supporting telehealth initiatives.
Key Responsibilities
- Assess basic health status and patient care needs for clinic visitors or callers.
- Analyze and interpret data to make informed patient care judgments.
- Evaluate outcomes of nursing interventions and update care plans accordingly.
- Collaborate with primary care providers and other care team members.
- Review refill requests per protocol, administer immunizations, medications, and other procedures, such as EKGs and wound care.
- Provide health education to promote patient self-management.
- Engage in nurse visits, telehealth, provider support, and administrative tasks.
Qualifications
- 1-2 years of direct patient care experience as an RN.
- Active MA RN license and BLS certification.
- Proficiency in EPIC software.
- Bilingual in Spanish or Portuguese is preferred.
- Previous experience in primary care is advantageous.
Skills
- Technical: EPIC software proficiency.
- Soft: Excellent communication and collaboration skills.
Additional Requirements
Monday to Friday schedule with varying shifts, including one evening shift on Wednesday and occasional Saturday rotations.
