Job Summary
Our client is seeking a dedicated Home Health Care Registered Nurse to take on a crucial role in improving patient care standards and community outreach. The primary responsibilities include program development, clinical oversight, patient care management, and ensuring compliance with regulatory standards.
Key Responsibilities
- Program Development: Set up office systems, create clinical forms, and establish workflows for the new skilled license.
- Clinical Oversight: Supervise LPNs, CNAs, and case managers, ensuring all services meet agency standards.
- Community Outreach: Conduct networking and marketing to build clientele, including hosting "mini health clinics" in the community.
- Patient Care Management: Perform medication assessments and ensure the execution of individualized care plans.
- Compliance & Safety: Participate in quality management and ensure adherence to State of Colorado regulations and accreditation standards.
- Staff Training: Provide staff development through educational programs and individual guidance.
Qualifications
- Current, valid license to practice as a Registered Nurse (RN) in Colorado or a compact state.
- Minimum of two (2) years of home health care experience is required.
- Current CPR certification and a valid driver’s license.
- Access to a reliable vehicle (though a company Nissan Leaf is available for business use).
- One year of supervisory experience is preferred.
