Compensation: $18.00 - $20.00 / hour (Depending on experience)
Schedule: Monday – Friday, 8:00 AM – 5:00 PM (On-site)

As a Care Navigator, you are the face of our clinic. This is a crucial, high-touch medical front desk role where you will guide our senior members and their families through their care journey from the moment they walk through our doors. Key Responsibilities

Patient Welcome & Check-In: Greet members warmly upon arrival, assisting them with electronic kiosk paperwork and navigating them through a seamless check-in process.

Administrative Coordination: Schedule appointments, coordinate multi-disciplinary care referrals, request medical records, and update the Electronic Medical Records (EMR) system.

Communication Management: Manage the clinic's phone lines by answering incoming calls, taking detailed messages, and routing inquiries from members, prospective patients, and their adult children.

Patient Education & Engagement: Provide basic patient education, answer community questions, and actively assist with hosting on-site center events.

Clinic Collaboration: Maintain a lean, efficient, and tidy front-office space, managing a steady flow of about 30 patients per day while balancing daily administrative tasks.
Required Skills & Qualifications

Experience: Minimum of 1 year of work experience in a clinical or healthcare setting.

Customer Service: Exceptional, high-level customer service skills with a warm, welcoming, and professional demeanor.

Medical Knowledge: Working knowledge of medical terminology, basic insurance processes, and/or standard clinic workflows.

Technical Savvy: Proficient PC skills (Microsoft Office Suite) and previous experience working with Electronic Medical Record (EMR) systems.

Preferred Qualifications

Direct experience utilizing eClinicalWorks (eCW).

An Associate’s degree or equivalent clinical experience.

Previous experience working directly with the geriatric/senior population.
