Job Summary
Our client is seeking a Referral Coordinator responsible for reviewing, processing, and verifying all elements necessary for an admission referral. The role involves coordinating referral management with various sources and internal teammates, maintaining program referral capacity, and ensuring compliance with federal and state regulations.
Key Responsibilities
- Reviewing, processing, and verifying all referral components for admission.
- Coordinating with referral sources and teammates to complete referrals.
- Managing program referral capacity and maintaining admission targets.
- Collaborating with internal teammates, referral sources, and patients/families.
- Engaging with referral portals for effective management.
- Applying and adhering to program policies, procedures, and regulations.
- Maintaining confidentiality of information.
Qualifications
- Active Licensed Practical Nurse/Licensed Vocational Nurse/Registered Nurse license.
- Experience in Home Health.
- Experience working with or knowledge of referrals.
- Preferred experience with Forcura and/or Home Care Home Base.
Skills
- Home Health Experience.
- Familiarity with referrals and related processes.
- Experience using referral management platforms like Forcura and Home Care Home Base is a plus.
Schedule/Shift
Monday to Friday, 9am-6pm or 10am-7pm CST.
