Job Summary

Our client is seeking a dedicated Pharmacist to play an integral role in assessing and determining patient suitability for home care, particularly in terms of prescribed medication therapy. The role involves ensuring the safe, appropriate, and effective delivery of pharmaceutical care with a focus on infusion and specialty products.

Responsibilities / Job Duties

• Assess and determine the suitability of patients for home care, organizing all necessary patient-specific information.
• Provide oversight of the safety, appropriateness, and effectiveness of the pharmaceutical care related to infusion and specialty products.
• Initiate drug therapy assessments, develop patient care plans and drug monitoring regimens, and monitor patients throughout therapy.
• Communicate with patients, physicians, nursing staff, pharmacists, and healthcare team members to ensure clinical outcomes are achieved.
• Compound intravenous and specialty pharmaceuticals aseptically as per guidelines, and handle sterile and hazardous medications appropriately.
• Ensure compliance with all regulatory standards, policies, and procedures.
• Provide direct oversight of pharmacy technicians and support staff.

Minimum Education and Experience Qualification Requirements

Certifications

• California Pharmacist License

Experience

• Multiple years of pharmacist experience, specifically in the specialty pharmacy space or hospital clean room environment.
• Preferred experience in the home infusion space with knowledge of USP 797 standards and sterile compounding.
