Job Summary
Our client is seeking a compassionate and self-motivated IRIS Consultant to support older adults and individuals with disabilities in achieving their goals and maintaining independence within their communities. In this role, you will serve as a trusted resource for participants, helping them navigate services, maintain program eligibility, and develop person-centered plans that reflect their unique needs and preferences.
This position offers a high degree of autonomy, flexibility, and the opportunity to make a meaningful impact in the lives of individuals and families throughout the community.
Key Responsibilities

Maintain regular participant outreach through monthly contacts, quarterly visits, and additional communication based on individual needs.

Build strong, collaborative relationships with participants through consistent communication and person-centered support.

Educate participants on program requirements and provide ongoing assistance to support eligibility and engagement in services.

Develop individualized Support and Service Plans in partnership with participants and their chosen support teams.

Collaborate with interdisciplinary teams, peers, and community partners to support participant goals and outcomes.

Complete required assessments, annual reviews, participant visits, and documentation in accordance with state and program requirements.

Document participant interactions, guardian communications, and collateral contacts within established timeframes.

Maintain accurate and organized records while effectively managing a caseload.

Monitor participant needs and ensure continuity of services through proactive communication and follow-up.
Qualifications
Required
-
Bachelor's degree in Social Work, Psychology, Human Services, Counseling, Nursing, Special Education, or a related field.

At least one of the following:
-
One year of supervised experience supporting older adults and/or individuals with disabilities.

Four or more years of direct social services experience working with these populations.

Current IRIS Consultant status in good standing with a DHS exemption.

Valid driver's license and reliable transportation.

Strong technology skills and comfort working within electronic documentation systems.

Understanding of human development, interpersonal dynamics, and community-based resources.
Preferred

Multilingual abilities, including Spanish, Hmong, Arabic, Russian, or other languages.
