Job Summary
Our client is seeking a dedicated Licensed Medical Social Worker to join their team. This role involves providing comprehensive social work services in a collaborative environment to ensure the well-being and care of residents within the facility.
Key Responsibilities
- Daily Clinical Collaboration: Attend the mandatory morning standup meeting every day at 9:45 AM alongside the clinical team, including the Director of Nursing, Director of Rehab, and the Assisted Living Manager.
- Documentation & Intake: Adhere to strict documentation deadlines, particularly for managing and processing new patient admissions.
- Patient Care Tracking: Utilize specialized, cloud-based software platforms such as Point Click Care, Netsmart, and Privet to accurately track patient care and facility metrics.
- Unit Coverage: Focus primarily on managing the caseload for the skilled nursing unit, which has a licensed capacity of 35 beds and a fluctuating census of about 15 to 27 residents.
- Cross-Community Support: Provide occasional, as-needed social work support to the independent living side of the community.
- Weekend Catch-up: Ensure all weekend activities, admissions, and pending items are thoroughly addressed and caught up during the mandatory Monday shift.
Qualifications
- Active Maryland LMSW license.
- Master's degree required.
- Proficiency in Point Click Care or similar cloud-based EMAR platforms.
- Skilled Nursing Facility (SNF) or Senior Living / CCRC experience preferred.
Skills
- Technical: Proficiency in Point Click Care, Netsmart, or Privet.
- Soft Skills: Strong communication and collaboration abilities.
