Job Summary
Our client is seeking a Licensed Practical Nurse responsible for overseeing and coordinating the care provided by Home Health Aides (HHAs) and Personal Care Aides (PCAs). The primary goal is to ensure that residents receive high-quality, individualized care in accordance with established care plans.
Key Responsibilities
- Partner with the Director of Health Care to supervise HHAs and PCAs, ensuring compliance with policies and quality standards.
- Maintain communication with insurance care teams and nursing supervisors to coordinate patient care.
- Manage staff assignments and daily operations to deliver high standard care to residents.
- Lead in resident assessments, care planning, and evaluation of nursing services.
- Promote resident-centered environment focused on wellness and independence.
- Engage in resident admissions, discharges, and transfers.
- Plan and conduct staff training and meetings.
- Ensure accurate implementation of physician orders.
- Coordinate physician appointments and necessary follow-up care.
- Oversee medication administration and treatment procedures.
- Meet with the Director to discuss care status and operational needs.
- Plan workforce scheduling to maintain care consistency.
- Communicate with families regarding health changes and recommendations.
- Ensure compliance with infection prevention protocols.
- Maintain emergency preparedness knowledge and educate staff.
- Foster a collaborative team environment.
- Perform additional clinical and administrative duties as needed.
Qualifications
- Graduate of an accredited nursing program with a current New York State nursing license and CPR certification.
- Prior experience in geriatric or long-term care nursing preferred.
- Ability to administer and document medications safely.
- Strong interpersonal and communication skills.
- Basic computer proficiency and electronic documentation experience preferred.
- Commitment to safe patient handling practices.
- Strong organizational and prioritization skills.
- Valid New York State driver's license preferred.
Skills
- Strong experience with dementia patients.
- Ability to stand, walk, and perform physical demands of the role.
- Flexibility and adaptability in improving processes.
- Ability to work weekends as needed.
- Proven relationship-building skills with residents and staff.
Additional Requirements
Ability to work a schedule of Monday to Friday from 11 PM to 7 AM, with a 40-hour workweek. Working every other weekend is a plus.
