Job Summary
Our client is seeking an Advanced Practice Nurse responsible for conducting in-home assessments and some outpatient clinical work on health plan members. The main goal is to ensure the effective management of patients, particularly through comprehensive assessments, chronic disease management, and education to patients and their families.
Key Responsibilities
- Conduct in-home assessments on health plan members, with occasional outpatient clinical work.
- Perform comprehensive history taking, physical exams, medication reviews, and cognitive/depression/safety screenings.
- Facilitate end-of-life care discussions, focusing on the geriatric patient population.
- Collect laboratory specimens in the home setting when appropriate.
- Identify diagnoses for active medical and care management of patients, emphasizing chronic disease management.
- Communicate findings to the primary care provider to highlight potential gaps in care.
- Deliver patient, family, and caretaker education, emphasizing monitoring and follow-up of patient needs.
- Utilize knowledge of appropriate community resources for referrals.
- Ensure compliance with HIPAA regulations and maintain the security of protected health information (PHI).
Qualifications
- Master’s degree from an accredited NP or PA program.
- Active North Carolina state Nurse Practitioner or Physician Assistant license.
- Active RN License and Furnishing number (required for NP upon hire).
- Active Nurse Practitioner Board Certification/Physician Assistant Certification.
- NPI Number and valid BLS certification.
- Valid North Carolina driver's license and current automobile insurance.
- Telemedicine experience is a must.
- 1+ years of clinical experience.
- Experience in Home Visits, Geriatrics, Medicare Advantage, or Wellness Checks (CMS requirements) is highly preferred.

- Minimum of 1+ years of clinical experience required.

- Clinical Skills: Comprehensive medical assessment and diagnostic skills.
- Soft Skills: Strong communication and interpersonal skills, particularly in patient and family education.
Additional Requirements
- Must be comfortable with end-of-life care discussions due to the geriatric patient population.
- May be required to work in home and outpatient settings.
