Job Summary
Are you a compassionate practitioner dedicated to empowering vulnerable adults and children? Our client is searching for Full-Time Licensed Registered Nurses (RNs) with reliable transportation to join our Community Personal Assistance Services (CPAS) Nurse Monitoring team. The role involves providing in-home assessments, developing care plans, and coordinating/monitoring services to determine the proper level of care for functionally and chronically disabled adults and children at risk of institutionalization.
Key Responsibilities
- Provide nurse monitoring services to individuals receiving personal care assistance and other services from home and community-based programs.
- Complete nursing assessments in the participant's home.
- Collaborate with the participant's Supports Planner, family members, and clinicians to address the participant’s needs.
- Ensure all participants receive the appropriate care plan based on their services.
- Document and report any concerns about quality of care or neglect.
Qualifications
- Valid State of Maryland RN license.
- Ability to pass a thorough background investigation.
- Possession of a valid driver's license equivalent to a noncommercial, class C Maryland driver's license at the time of appointment.
