Compensation: $18.00 – $22.00 / hour (dependent on experience)
Schedule: Monday – Friday, 8:00 AM – 5:00 PM (Punctuality is crucial as daily meetings begin promptly at 8:00 AM)
Locations: Largo, FL (33764) & St. Petersburg, FL (33710)
(This is a float role covering two neighborhood clinic locations situated about 20 minutes apart. No travel is required during the middle of the workday.)
Job Summary
An innovative, senior-focused healthcare organization is seeking a dynamic and adaptable Care Navigator to join a growing clinical team. This role essentially functions as a medical front desk position within a lean, collaborative neighborhood clinic setting. While the patient volume is highly personalized (averaging around 10 patients per day), you will stay consistently engaged with administrative tasks, provider coordination, and phone management.
As the primary point of contact, you will serve as a guide for members and their families, ensuring every senior patient experiences a welcoming, seamless, and high-quality visit from the moment they walk through the door.
Core Responsibilities

Member Greeting & Check-In: Welcome senior members warmly upon arrival, helping them feel safe and comfortable while navigating them through the check-in process.

Patient Navigation: Assist patients with filling out administrative paperwork via electronic kiosks as needed.

Scheduling & Referrals: Manage appointments and efficiently coordinate medical referrals across a multi-disciplinary clinical care team.

EMR Management: Utilize, manage, and update member information within the Electronic Medical Records (EMR) system.

Medical Records Processing: Request incoming medical records and systematically upload external clinical documentation into the EMR platform.

Communication Management: Oversee the clinic phone lines by answering incoming calls, fielding questions from members and their adult children, taking detailed messages, and routing information to the correct internal staff.

Patient Support & Education: Deliver baseline patient education and guide members through center workflows.

Community Engagement: Assist with organizing, setting up, and managing community center events as needed.
Skills & Qualifications
Required Qualifications

Healthcare Experience: Minimum of 1 year of work experience in a healthcare or clinical setting.

Geriatric Care Passion: A genuine passion for supporting and interacting with the geriatric/senior population.

Adaptability: Ability to thrive and remain flexible in a rapidly evolving, lean organizational environment where training and protocols can update on the fly.

Technical Literacy: Working knowledge of basic medical terminology, health insurance concepts, and digital computer systems (including PC skills and Microsoft Office Suite).

Teamwork & Accountability: A reliable team-player mentality with excellent communication skills, strong attention to detail, and top-tier punctuality.
Preferred Qualifications

eClinicalWorks (eCW): Direct, hands-on experience utilizing eClinicalWorks software is highly preferred.

Education: Associate's degree or equivalent professional clinical experience preferred.
