Title: Social Worker - QMHP

Location: Boardman, OR 

Facility: Community Health / Outpatient Clinic 

Duration: 26 Weeks

Start Date: ASAP

Shift/Schedule: Monday–Friday, 8:00 AM – 5:00 PM (Potential for 4x10 schedule upon clinician request)

Responsibilities:

• Provide lifespan therapy (adolescents through geriatrics) in an outpatient office setting, handling both standard and higher-volume caseloads.
• Manage long-term therapy cases, including patients who may have initially entered care in a crisis state.
• Collaborate with community partners, including schools, DHS, local courts, and law enforcement.
• Participate in all required clinical morning meetings and daily team discussions.
• Maintain accurate and timely documentation using the Credible EMR system.

Required Skills:

• Active Oregon Clinical License
• Fluent Bilingual Proficiency in Spanish and English
• Previous relevant clinical experience preferred
