Job Summary
Our client is seeking a Clinical Nurse Specialist/Registered Nurse with a primary focus on transitioning patients from inpatient to home care. The role involves promoting home infusion services, engaging with patients and facility staff, and ensuring smooth transitions in care. As a sales professional, the candidate will help grow referral volume and maintain strong relationships with referral sources.
Key Responsibilities
- Transitioning patients from inpatient to home care, without entering their homes.
- Act as a sales professional promoting home infusion services to discharging patients.
- Educate patients, families, and facility staff on home infusion services.
- Assess patients for eligibility for home/alternate site care.
- Coordinate care transitions and ensure seamless discharge planning.
- Build and maintain relationships with referral sources, such as case managers and providers.
- Collaborate with Account Managers to increase referral volume and meet sales goals.
- Review medical records and gather pre-admission and insurance information.
- Facilitate communication across care teams and ensure treatment plan execution.
- Support patient education and therapy independence.
- Coordinate logistics for supplies, orders, and care setup.
Qualifications
- Bachelor's degree in Nursing.
- Licensed RN in the state of practice.
- Minimum 2+ years of nursing experience.
- Experience in building relationships.
- Knowledge of healthcare standards (ACHC, URAC, etc.).
- Strong customer service experience.
- Basic Microsoft Office skills (Word, Excel, PowerPoint).
- Ability to identify and improve operational processes.
- Reliable transportation with a valid driver’s license and insurance.
Additional Requirements
- Schedule: Monday to Friday, 8:00 am to 5:00 pm, no weekends.
- No on-call duties.
