Job Title: Assistant Director of Community Engagement, Partnerships, and Program Operations - Jericho, NY or Remote
Ideal Background: Individual with a strong ESRD/Nephrology background but is interested in building programming and education material as well as working with a number of organizations and stakeholders.
Responsibilities:
This National AD position oversees and implements all KHARES activities which are aimed at building community engagement and partnerships across the nation. These activities include planning, hosting and evaluating all monthly key priority calls. Working within the community to gather best practices and develop key priority resources and educational tools. Outlining and implementing processes to reach newly diagnosed ESRD patients with resources, as well as supporting training and strategic communications priority events. The Director will work in collaboration with CMS, the ESRD Networks and community leaders and other stakeholders across the nation to define goals and oversee the design, implementation, metric development and monitoring, rapid cycle improvement, and evaluation of progress throughout the contract period. He/she will work with the KHARES national analytics leads to ensure data is converted to consumable analytics for the end user and highlight high impact triggers for data monitoring. The individual will also be responsible for managing the “ESRD track” for quality conference ensuring CMS expectations for content delivery and execution are met each year. The candidate will have strong verbal and written communication skills, be able to interpret and utilize data to drive forward quality ESRD Network improvement goals and be proficient in use of tools in the Microsoft Office Suite as well as ChatGPT and Co Pilot to support communications, public presentation, engagement and data analysis, and program implementation evaluation.
- Develops and implements monthly CMS/ESRD Network workgroup calls to share best practices, successful quality improvement interventions and highlight technical assistance efforts across the Network key priority areas.
- Connects groups of community providers with specialists at centers of excellence to offer case-based learning opportunities to help local workers gain expertise. These calls will be made in partnership with the ESRD Networks to build a collective experience.
- Develops/maintains/updates Change Packages to provide practical strategies and tactics for QI at dialysis facilities for every ESRD Network goal area. Change Packets will be developed utilizing information obtained through collaboration with high performing facilities and review of research as well as evidence-based tools available in the community.
- Using HCD the Lead will work with facilities, patients, family members and/or caregivers to develop patient educational resources focused on the key priority areas.
- Works to keep new patient orientation materials updated and accessible to improve the reach and engagement with these materials through the ESRD Networks.
- Supports the development of ESRD Network training on key priority measure specifications, and EQRS releases.
- Hosts strategic communications through quarterly Town Halls on ESRD on a wide range of ESRD topics
- Supports the PFE Learning and Action Network activities assisting with recruiting, resource development, and meeting planning.
- Leads national conference planning and logistics to ensure all CMS expectations are met for the annual quality conference serving on the larger CMS quality conference planning team and supporting hosting ESRD subcommittee planning meetings to host the ESRD track.
- Recruits and prepares speakers for identified ESRD topics including working with PFE lead to identify patient speakers
- Supports logistic planning for meeting offerings as well as for invited guests working in collaboration with the Emergency Lead on the Emergency Summit which coincides with the meeting.
- Develops and provides CMS Quality Conference Summary that outlines key points and lessons learned, as well as other materials that can be shared with other patients and families, to spread the knowledge they gained and empower them to improve kidney health.
- Develop presentations and provide administrative support to CMS staff attending ESRD-focused conferences, quality improvement symposiums, and stakeholder engagement meetings that directly advance CMS initiatives in dialysis facility oversight, patient safety, and clinical outcome improvements.
- Supports all aspects for general program management including monthly CMS meetings and progress reports, annual summary development, support for CMS hosted calls, and communication from CMS to the Networks.
- Develops white papers, position papers and presentations to enable CMS to make appropriate efficient and effective decisions.
- Tracks and monitors CIQIP and KHARES quality indicators to project work plan progress identifying trends and suggesting interventions where trends suggest potential barriers.
- Participates in the development of KHARES evaluation documents and participates in the evaluation session.
Qualifications:
- Strong communication skills (written and verbal) with the ability to develop clear and concise reports and effectively communicate with, and be understood by, persons contacting the Network via telephone
- Strong interpersonal and advocacy skills with the ability to deal professionally and successfully with a diverse population
- Strong problem solving and organizational skills with the ability to prioritize assignments, maintain detailed records, and meet deadlines
- Must be proficient in Microsoft Office Suite and have experience working with Excel and PowerPoint; knowledge of statistical analysis methodologies to analyze data.
- Knowledge of state/federal agencies, community resources, and nephrology are desirable.
- An understanding of continuous quality improvement concepts and tools
- Ability to abstract, compile, and interpret data.
- Prior research experience a plus
- Ability to travel as required.
Education/Experience:
- A Bachelor’s degree in a related discipline, required. A Master’s degree in a related discipline, preferred.
- A minimum of three (3) years’ experience in Nephrology; experience with all modes of treatment available to renal patients preferred.
- Professional certification preferred (e.g. certified nephrology nurse)
Schedule/Shift: Monday-Friday 40 hours EST
Location: Ideally the nurse sits in office in Jericho, NY but would consider remote for the right candidate
Pay: 100-120k
