Position Overview Medix is partnering with a leading national healthcare provider to find a dedicated, autonomous Nurse Practitioner to join their national after-hours care team. This is a fully remote, work-from-home position responsible for delivering telephonic and virtual care to residents in senior living and skilled nursing facilities during evenings, nights, weekends, and holidays.
In this role, you will serve as a key clinical resource, providing timely medical guidance and support to both residents and facility staff. You will play a critical role in managing acute concerns, reducing unnecessary hospital transfers, and ensuring continuity of care outside of standard business hours. This position requires comfort with moderate to high call volumes, independent decision-making, and conducting telehealth evaluations.
Compensation
Key Responsibilities

Serve as the first line after-hours clinical support to aging residents and facility staff via phone and telehealth platforms.

Assess and manage acute changes in condition, falls, chronic illness exacerbations, and behavioral health concerns.

Conduct virtual assessments (comprising approximately 30% of visits) and provide direction on next steps, safely treating patients in place when appropriate.

Coordinate care across settings, including direct communication with facilities, hospitals, and outpatient providers.

Perform comprehensive assessments and thoroughly document all encounters in the electronic medical record (GEHRIMED) in compliance with regulatory and CMS standards.

Comfortably handle an average of 4–5 calls per hour, utilizing internal nursing triage support to prioritize urgent concerns.
Schedule and Availability

Part-time commitment of approximately 24 hours per week with a consistent, set schedule.

Coverage windows include weeknights (5:00 PM – 8:00 AM) and continuous weekend coverage (Friday at 5:00 PM to Monday at 8:00 AM).

Rotational weekend coverage: Shift options generally consist of 12-hour shifts (6:00 AM – 6:00 PM or 9:00 AM – 9:00 PM) every other weekend.

Weeknight shifts typically run from 6:00 PM – 12:00 AM or 6:00 PM – 6:00 AM as needed.

Holiday coverage rotation is required, consisting of two 12-hour holiday shifts per year scheduled in advance.
Required Qualifications

Master’s degree from an accredited Nurse Practitioner program.

National board certification as an Adult Nurse Practitioner (ANP), Family Nurse Practitioner (FNP), or Gerontological Nurse Practitioner (GNP).

Minimum of 3 years of clinical experience practicing as a Nurse Practitioner (new graduates cannot be considered).

Active, unrestricted Nurse Practitioner license in Iowa (IA), Missouri (MO), and Kansas (KS) at the time of application, with a willingness to obtain additional state licenses as needed.

Active DEA licensure and prescriptive authority to manage controlled substances.

Demonstrated experience managing acute and chronic conditions in adult or geriatric populations.

High proficiency with electronic medical records (EMR) and strong independent clinical decision-making skills.
Preferred Qualifications

Prior experience working in skilled nursing facilities, long-term care, senior living, urgent care, or emergency medicine environments.

Deep understanding of geriatric care, chronic disease management, advanced illness care, and end-of-life discussions.

Experience with telehealth or telephonic clinical triage.

Adaptable, resilient communication skills with the ability to build rapport with remote interdisciplinary teams and facility staff under time pressure.
