Job Summary
Our client is seeking a Referral Coordinator to manage a caseload of 10–25 files, coordinate safe hospital discharges, find and negotiate with home health providers, and ensure the accuracy of demographic data. This role also involves promoting positive public relations and assisting with marketing calls.
Key Responsibilities
- Manage a caseload and coordinate safe discharges from hospitals.
- Provide staffing for home health and IV therapy referrals.
- Ensure the accuracy of demographic data and maintain excellent follow-up with involved parties.
- Negotiate with vendors to maintain appropriate pricing and margins.
- Support the mission and values of our client through excellent customer service and compliance with regulatory requirements.
- Assist with on-call duties during assigned weekends.
- Maintain reliable attendance and comply with information security policies.
Qualifications
- Some college education and/or medical background; LPN preferred.
- Experience in Work Comp Insurance, referrals, and authorizations.
- Proficient in MS Office, Word, Excel, and PowerPoint.
- Excellent organizational and prioritization skills.
- Strong interpersonal and communicative skills.
- Ability to work cohesively with management, co-workers, and external customers.
Skills
- Proficient reading, writing, grammar, and mathematics skills.
- Knowledge of medical terminology.
- Moderate PC skills with an emphasis on accuracy.
- Bilingual in Spanish preferred.
