Job Summary
Our client is seeking a Clinical Coordinator/Registered Nurse to join their team. The primary role includes assessing and collecting patient health data, executing nursing interventions, and coordinating multidisciplinary healthcare services.
Key Responsibilities
- Assess and collect data related to patient health status and input this medical data into the EMR as instructed by the physician.
- Collaborate with the care team to develop and implement care plans across the healthcare continuum, including medication management and prior authorizations.
- Administer immunizations and medications, and execute nursing interventions under limited supervision.
- Support the ongoing clinical functions to provide compassionate patient-centered care.
- Handle telephone, MyChart, and in-person triage for symptom management and patient education.
- Assist with urgent and emergent patient situations in collaboration with care team leadership.
- Coordinate diagnostic tests and perform procedures under physician supervision.
- Review and act on test results and manage internal and external communications.
- Ensure infection control compliance within the practice.
- Provide patient and family satisfaction, float to other clinics as needed, and precept new clinical staff.
Qualifications
- Bachelor's degree in Nursing or equivalent in education and experience.
- BLS certified.
- Current New York State RN license.
- 2-4 years of related experience.
Experience
- Minimum 2-4 years of experience in a nursing role.
- Experience in Pediatrics.
