Job Summary
Our client is seeking a Licensed Clinical Social Worker to identify, triage, and manage patients with mental health and behavioral health conditions. The role involves collaborating with a team of primary care physicians and mid-level providers to enhance patient care through skill training, patient education, and specific behavioral change plans.
Responsibilities / Job Duties
- Identify and manage patients with mental and behavioral health conditions within a primary care setting.
- Provide skill training and develop behavioral change plans with patients.
- Assist primary health care providers in recognizing and treating mental disorders and psychosocial problems.
- Assess clinical status and detect "at risk" patients to prevent further deterioration.
- Consult with physician supervisors and refer cases to specialty providers as needed.
- Collaborate with the primary care team to manage patients with chronic emotional and/or health problems.
- Improve clinical outcomes for patients with high prevalence of mental disorders.
- Teach self-management, prevention, and treatment techniques to patients, families, and staff.
- Maintain accurate patient records and supervise case coordinators.
- Participate in meetings, Quality Assurance activities, and staff evaluations.
Minimum Education and Experience Qualification Requirements
Certifications
- Must have a current CA license.
Skills
- Keen understanding of behavior modification strategies, motivational interviewing, and value-based strategies.
- Experience in crisis work or solution-focused therapy is preferred.
- Strong teamwork abilities and excellent interpersonal, written, and verbal communication skills.
- Ability to handle confrontational situations calmly and equitably.
- Ability to read, analyze, and interpret business periodicals, professional journals, and governmental regulations.
Schedule / Shift
Monday to Friday, 8am-5pm. Flexible start times are available, maintaining standard 8-hour days.
