Job Summary
This is a unique opportunity for a community-minded Contract Registered Nurse to join a multidisciplinary team providing specialized care to older adults with complex mental health needs. As the medical anchor of the team, our client seeks a professional who will move beyond the clinic walls to deliver life-changing care directly to clients within their community.
Key Responsibilities
- Conduct integrated mental health and basic medical assessments to build individualized, person-centered care plans.
- Administer psychiatric medications, monitor for compliance, and track side effects in collaboration with psychiatric providers.
- Deliver evidence-based nursing treatments and rehabilitative services tailored to the geriatric population.
- Act as the primary link between clients, primary care physicians, and community housing agencies to ensure a seamless continuum of care.
- Participate in de-escalation and crisis intervention to maintain client safety and stability in their home environment.
- Provide health literacy training to clients and families regarding chronic conditions, medication, and wellness strategies.
- Lead medical discussions in daily ACT morning meetings and case conferences to ensure integrated service delivery.
Qualifications
- Active and unencumbered Registered Nurse (RN) license in the State of New York.
- Strong understanding of psychiatric medications and their interactions within an aging population.
- Proficiency in navigating Electronic Health Records (EHR) and maintaining meticulous clinical notes.
- Ability to perform rapid assessments and interventions during mental health crises.
- Can be a newer RN looking for a community-based role.
- Prior experience specifically working with older adults (65+) or an understanding of the intersection between aging and chronic mental illness.
- Fluency in a second language (such as Spanish) to better serve the diverse population.
- Familiarity with the NYC social services landscape, including housing vouchers, Medicare/Medicaid navigation, and local community resources.
- Current certification in crisis de-escalation techniques (e.g., CPI or Mandt).
- Background in inpatient psychiatry or previous experience on an ACT/FACT team.

Additional Requirements
- This is a community-based role requiring travel within the service area.
- While standard hours are Monday–Friday (9:00 AM – 5:00 PM), the team operates on a 4-day field-based rotation. Flexibility is required to meet client needs, which may occasionally include weekends or holidays.
