Care Manager – Social Work (LMSW/LCSW)

Position Details:
- Location: Remote (Orientation held in the Bronx, NY)
- Schedule: Monday–Friday | 8:00 AM – 5:00 PM (1-hour lunch)
- Pay Rate: $39–$40/hour
- Contract Length: LOA Coverage from 5/15/2026 – 9/21/2026 (subject to change)
Position Overview:
Responsible for overall care management and quality of care for participants. Uses specialized discipline-specific knowledge to review assessments of field staff and coordinate a holistic care plan that addresses all domains of care.
Key Responsibilities:
- Participate in care planning meetings and represent the social work discipline as needed
- Review discipline-specific documentation for quality and address deficiencies with field staff
- Monitor field staff documentation of interventions and provide feedback or corrective action as needed
- Conduct coaching sessions with field staff to support performance and compliance
- Communicate regularly with field staff to coordinate a continuum of care aligned with members’ needs and goals
- Develop and manage holistic care plans to support optimal health and functional outcomes
- Coordinate with the interdisciplinary team (IDT) to arrange and authorize appropriate services (e.g., ADLs, IADLs, housing, meals, transportation)
- Implement care management interventions to help participants achieve their goals
- Collaborate with team members, facilities, discharge planners, and home care coordinators during care transitions
- Document all services in compliance with organizational standards and federal/state regulations
- Coordinate long-term care services (e.g., nursing homes, rehab facilities) as needed
- Partner with PCPs, specialists, and IDT members to address changes in participant conditions and adjust care plans accordingly
- Arrange or provide ongoing skilled services, authorizations, and periodic reassessments
- Monitor effectiveness of care plans and adjust services based on participant progress and outcomes
- Evaluate care plans and modify as needed to meet changing health needs and goals
- Ensure cost-effective service delivery aligned with member needs
- Maintain up-to-date knowledge of healthcare practices, guidelines, and regulations
- Conduct staff trainings and competency evaluations as needed
- Recommend improvements to programs, policies, and procedures
- Follow organizational policies related to disciplinary actions and collaborate with HR when needed
- Act within scope of practice and maintain required competencies
- Serve as an active member of the interdisciplinary team (IDT)
- Perform other duties as assigned
Qualifications:
Education
- Master’s Degree in Social Work from a program approved by the New York State Education Department (NYSED)
License
- Active, unrestricted New York State Social Work license and registration required:
Experience
- Minimum of 2 years of administrative experience in one or more of the following:
- Certified Home Health Agency (CHHA)
- Managed Care
- Long-Term Home Health Care (LHCSA)
- Acute Care
- Medical-Surgical/Critical Care
- Nursing Home
- Diagnostic/Treatment Clinic
- Customer service experience required
- Managed long-term care insurance experience preferred
- Supervisory experience preferred
- Minimum of 1 year working with a frail or elderly population
- Candidates with less than 1 year must complete PACE-specific training upon hire
