Registered Nurse (RN) – Home Health Field Nurse
- Denver, CO & Northern Colorado
- $80,000–$85,000 + $5,000 Sign-On Bonus
- Full-Time | Monday–Friday | 8AM–5PM | No Weekends
We are seeking a compassionate and motivated Registered Nurse (RN) to join a growing healthcare organization supporting patients throughout the Denver and Northern Colorado market. This is a field-based role focused on home care support, care plan development, supervisory visits, and care coordination for patients receiving home-based services.
This opportunity is ideal for an RN who enjoys working independently, building relationships with patients and providers, and making a direct impact within the community.
Responsibilities

Develop and maintain patient care plans supporting authorizations and renewals

Conduct in-home supervisory and compliance visits throughout the assigned region

Support patients with start-of-care and continued authorization processes

Build positive relationships with patients, case managers, providers, and care teams

Ensure accurate and timely clinical documentation while maintaining HIPAA compliance

Escalate critical incidents appropriately and in a timely manner
-
Collaborate cross-functionally with clinical, care management, and pre-care teams

Manage a field-based schedule while providing exceptional patient-centered care
Qualifications
-
Active and unrestricted RN license in the state of Colorado

2+ years of RN clinical experience required

Home health or home care experience preferred

Comfortable conducting daily in-home patient visits

Strong communication, organizational, and documentation skills

Ability to work independently in a field-based environment

Proficiency with Microsoft Office and clinical documentation systems

Experience in fast-paced or startup healthcare environments is a plus
