Job Title: Remote After-Hours Nurse Practitioner (NP)
Location: Fully Remote (Must hold active licenses in New York & Pennsylvania)
Schedule: Part-Time | ~24 Hours/Week | Evenings, Nights, Weekends, Holidays
Overview
We are seeking an experienced and highly autonomous Nurse Practitioner to join a national after-hours care team supporting residents in senior living and skilled nursing settings. This fully remote role plays a critical part in delivering high-quality, timely care outside of standard business hours—helping reduce unnecessary hospitalizations while ensuring continuity of care.
You will serve as a key clinical resource, providing telephonic and virtual care to both patients and facility staff in a fast-paced, high-volume environment.
Key Responsibilities
- Deliver after-hours clinical support via phone and telehealth platforms
- Assess and manage acute, chronic, and behavioral health concerns
- Evaluate changes in condition, including falls and medication-related issues
- Collaborate with physicians and interdisciplinary care teams
- Coordinate care across facilities, hospitals, and outpatient providers
- Document all encounters accurately in the EMR in compliance with regulatory standards
- Apply clinical protocols to support safe, effective “treat-in-place” care when appropriate
- Participate in ongoing training and quality initiatives
Schedule & Availability
- Approximately 24 hours per week with a consistent, set schedule
- Weeknights: 5:00 PM – 8:00 AM
- Weekends: Friday 5:00 PM – Monday 8:00 AM
- Saturday & Sunday shifts split into 12-hour blocks (6:00 AM – 6:00 PM)
- Holidays: Coverage required (12-hour shifts)
- Requirement: Minimum of two 12-hour holiday shifts annually
- Note: Overnight, weekend, and holiday participation is required
Required Qualifications
- Master’s degree from an accredited Nurse Practitioner program
- Active, unrestricted NP licenses in both New York and Pennsylvania
- National certification (ANP, FNP, GNP, or equivalent)
- Eligibility for prescriptive authority and DEA licensure
- Minimum 3 years of clinical NP experience
- Experience managing acute and chronic conditions in adult or geriatric populations
- Ability to work independently and manage multiple patient interactions simultaneously
- Proficiency with EMR systems and clinical documentation

- Experience in skilled nursing, senior living, or post-acute care settings
- Strong background in geriatric care and chronic disease management
- Experience with advanced illness care and end-of-life discussions
