Job Summary
Our client is seeking a dedicated Dietitian to conduct nutritional assessments, develop dietary recommendations, and educate individuals on nutrition principles. The role involves working with interdisciplinary teams to provide comprehensive, participant-centered care and support the health, independence, and quality of life of older adults through individualized nutritional care.
Key Responsibilities
- Conduct initial, annual, and ongoing nutritional assessments to identify participant needs and support individualized care planning
- Develop and implement dietary recommendations and nutrition interventions in collaboration with providers and interdisciplinary care teams
- Educate participants, families, caregivers, and support partners on nutrition principles, meal planning, food preparation, and dietary modifications
- Assist participants and caregivers in identifying seasoning and meal preparation methods that align with dietary restrictions while supporting taste preferences
- Monitor participant progress and communicate changes in nutritional status, eating habits, and weight trends to the care team and providers
- Collaborate with interdisciplinary team members to coordinate care and address potential nutrition-related concerns proactively
- Provide nutrition-related support and education across multiple care environments including homes, hospitals, nursing facilities, and assisted living settings
- Manage nutritional supplements and specialized dietary needs or restrictions as appropriate
- Maintain accurate, timely, and compliant documentation within the participant medical record
- Participate in interdisciplinary team meetings and contribute to ongoing treatment planning and care coordination
- Support the oversight and coordination of dietary and food service programming within the center environment
- Assist with quality improvement initiatives, policy development, clinical protocols, and organizational service programs as requested
- Manage assigned caseload responsibilities while maintaining cost-effective and quality-focused care delivery
- Utilize organizational resources responsibly and maintain compliance with established operational and budgetary guidelines
- Participate in orientation and education activities for staff, students, and volunteers as needed
- Perform additional duties as assigned
Qualifications
- Bachelor’s degree in Nutrition, Dietetics, or a related field from an accredited institution required
- Active Registered Dietitian credential through the Commission on Dietetic Registration required
- Minimum of one year of experience working with geriatric populations, home care, long-term care, or related healthcare settings preferred
- Valid Colorado Driver’s License and proof of auto insurance

- Strong understanding of nutritional assessment, dietary planning, and clinical nutrition principles
- Ability to work effectively with culturally, economically, and educationally diverse populations
- Excellent communication and interpersonal skills with the ability to collaborate across interdisciplinary teams
- Strong organizational skills and ability to manage multiple priorities in a clinical setting
- Knowledge of medical documentation standards, confidentiality practices, and participant-centered care approaches
