Job Summary
Our client is seeking a Clinical Pharmacist to join their team. The primary responsibilities include assessing patient suitability for home infusion therapy, designing and monitoring pharmacy care plans, recommending medication therapies, and preparing medications according to standards. The role aims to ensure the highest quality of pharmaceutical care in a collaborative and patient-focused environment.
Key Responsibilities
- Assess patient suitability for home infusion therapy
- Design, monitor, and evaluate individualized pharmacy care plans
- Recommend or adjust medication therapies based on clinical data
- Prepare, compound, label, and dispense medications per USP and DEA standards
- Conduct nutritional assessments and provide patient/caregiver education
- Collaborate with nursing and other departments to ensure continuity of care
- Monitor controlled substance inventory and report discrepancies
- Provide drug information to healthcare professionals and patients
- Participate in CEU and professional competency programs
- Adhere to company policies, quality assurance, and regulatory requirements
Qualifications
- Graduate of an accredited School of Pharmacy
- Active Pharmacy License in the State of Texas
- 1+ year experience in sterile IV admixture procedures, knowledge of non-sterile compounding, immunizations, and MTM or completion of an ASHP-accredited Pharmacy Residency Program
- Proficient in Microsoft Word, Excel, and PowerPoint
- Strong customer service and patient-care orientation
- Preferred: Experience in home care, hospital, or clean room settings and indirect management experience

Additional Requirements
The work schedule includes shifts from 9:30 a.m. to 6:00 p.m., with on-call rotations from 10:30 a.m. to 7:00 p.m., occurring every 7th weekend and about three times a month during the week.
