Job Summary
Our client is seeking a dedicated Nurse Practitioner to provide advanced clinical care to patients. This role involves assessing, diagnosing, treating, and managing both acute and chronic conditions. Working independently and collaboratively with physicians and healthcare teams, the Nurse Practitioner will deliver high-quality, patient-centered care while ensuring compliance with clinical guidelines and regulatory standards.
Key Responsibilities

Advanced Clinical Assessment: Perform comprehensive physical examinations and detailed health history evaluations to establish a baseline for expert patient care.

Diagnosis & Chronic Care Management: Independently diagnose and manage a diverse range of acute, chronic, and complex health conditions using evidence-based clinical judgment.

Treatment Planning & Execution: Design, execute, and adapt individualized therapeutic plans, including the prescription of medications and treatments in strict accordance with state licensure.

Diagnostic Interpretation: Direct the ordering and evaluation of diagnostic imaging and laboratory tests to inform and refine clinical decision-making.

Collaborative Care Coordination: Facilitate seamless continuity of care by partnering with interdisciplinary healthcare teams and managing strategic referrals to specialists when necessary.

Patient Advocacy & Counseling: Serve as a primary clinical educator for patients and their families, providing essential guidance on disease prevention and long-term health management.

Clinical Integrity & Documentation: Ensure the highest level of data accuracy by maintaining precise records within the EHR and adhering to all regulatory and privacy standards.
