Job Summary
Our client is seeking a dedicated Hospice Registered Nurse to provide professional nursing care to patients in their homes. This role involves assessing, planning, implementing, and evaluating individualized care needs on an ongoing basis. The nurse ensures that care is delivered in accordance with organizational policies, procedures, and applicable regulatory requirements.
Key Responsibilities
- Ensure timely execution of physician orders and maintain accurate documentation reflecting compliance and patient progress toward discharge goals.
- Deliver patient care in alignment with established policies, procedures, and standards of practice, ensuring safety and quality outcomes.
- Apply sound clinical judgment and critical thinking to support safe and effective patient outcomes, accurately documenting clinical decisions.
- Identify appropriate payer sources and ensure compliance with authorization requirements for all services.
- Complete all required documentation, including assessments, care plans, and interventions, within established timelines.
- Implement targeted patient education strategies aligned with care plan goals and document accordingly.
- Communicate patient care concerns promptly and serve as a liaison to support effective care coordination.
- Supervise home health aide staff and stay current with evolving clinical standards.
- Meet physical requirements, including lifting, pushing, and pulling up to 50 lbs., and standing for extended periods.
Qualifications
- Associate’s Degree in Nursing or Nursing Diploma.
- Minimum of six (6) months RN experience or one (1) year LPN experience, preferably in home care or hospice.
- Active New Jersey Registered Nurse License.
- Basic Life Support (BLS) for Healthcare Providers (AHA certification required).
- Basic computer proficiency.
