Job Summary

Our client is seeking a dedicated ICU Registered Nurse to join their dynamic primary care team. This pivotal role requires high-level triage, advanced clinical judgment, and effective care coordination, ensuring the delivery of top-tier patient care. As a key clinical leader, you will act as a primary liaison between patients and healthcare providers, playing a vital role in patient education and supporting medical staff competency.

Key Responsibilities

• Manage clinical coordination and virtual care, including medication refills, lab result interpretation, and telephonic triage.
• Lead direct patient care interventions focusing on high-risk management and chronic disease stabilization.
• Perform advanced clinical triage for scheduled and emergent visits.
• Identify and support rising risk patients through developed care plans.
• Provide clinical mentorship and validate competencies of medical staff.
• Optimize provider efficiency through complex paperwork management and specialty referrals.
• Educate patients on complex disease processes and medication regimens to foster engagement and self-management.

Qualifications

• Current Colorado RN License.
• Current CPR (BCLS) certification.
• Graduate of an accredited Nursing Program required; BSN preferred.
• Two years' experience in case management and clinical nursing preferred.
• Basic computer skills, including the ability to manage timecards, access policies, and use PowerPoint for presentations.

Skills

• Epic software proficiency is preferred.
• Bilingual in Spanish is preferred.
• Experience with Medicare/Medicaid population and paperwork is an asset.

Additional Requirements

Shift Schedule: Monday to Friday, no weekends. Options include working 4 days with 10-hour shifts (7:30am-6pm) or 5 days with 8-hour shifts (8am-5pm).
