Job Summary
Our client is seeking a compassionate and highly skilled Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) to join its outpatient Neuro-Oncology team. The role involves supporting patients and families navigating complex neurological cancers by providing psychosocial assessments, counseling, care coordination, and resource navigation throughout the continuum of care.
Key Responsibilities
- Manage a caseload
- Discuss the patients plan of health with them
- Coordinate Care
- Work with the clinical team
- Conduct comprehensive psychosocial assessments for patients diagnosed with neurological cancers
- Provide individual, family, and group counseling to support coping, adjustment, and mental health needs
- Develop and implement patient-centered care plans in collaboration with the interdisciplinary team
- Offer crisis intervention and emotional support for patients and caregivers
- Assist with advance care planning, goals-of-care discussions, and end-of-life support when appropriate
- Coordinate referrals to community resources, including financial assistance, transportation, home care, and support services
- Facilitate support groups and educational sessions for patients and caregivers
- Advocate for patients' needs within the healthcare system and community
- Maintain accurate and timely documentation in the electronic medical record
- Participate in multidisciplinary case conferences and team meetings
Qualifications
- Master’s Degree in Social Work (MSW) from an accredited program
- Active New York State LMSW or LCSW license required
- Minimum 1–2 years of clinical social work experience in a healthcare or oncology setting preferred
- Experience working with medically complex or oncology patient populations strongly preferred
- Strong assessment, counseling, and crisis intervention skills
- Knowledge of community resources and healthcare systems in NYC
- Excellent communication, organizational, and interpersonal skills
- Ability to work effectively in a fast-paced, team-based environment
Preferred Qualifications
- LCSW with psychotherapy experience
- Prior experience in oncology, neurology, palliative care, or hospice settings
- Bilingual (Spanish or other languages) a plus
Skills
- Previous Social Work experience
- Proficiency with EPIC electronic medical records preferred
