Job Summary

Our client is seeking a compassionate and highly skilled Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) to join its outpatient Neuro-Oncology team. The role involves supporting patients and families navigating complex neurological cancers by providing psychosocial assessments, counseling, care coordination, and resource navigation throughout the continuum of care.

Key Responsibilities

• Manage a caseload
• Discuss the patients plan of health with them
• Coordinate Care
• Work with the clinical team
• Conduct comprehensive psychosocial assessments for patients diagnosed with neurological cancers
• Provide individual, family, and group counseling to support coping, adjustment, and mental health needs
• Develop and implement patient-centered care plans in collaboration with the interdisciplinary team
• Offer crisis intervention and emotional support for patients and caregivers
• Assist with advance care planning, goals-of-care discussions, and end-of-life support when appropriate
• Coordinate referrals to community resources, including financial assistance, transportation, home care, and support services
• Facilitate support groups and educational sessions for patients and caregivers
• Advocate for patients' needs within the healthcare system and community
• Maintain accurate and timely documentation in the electronic medical record
• Participate in multidisciplinary case conferences and team meetings

Qualifications

• Master’s Degree in Social Work (MSW) from an accredited program
• Active New York State LMSW or LCSW license required
• Minimum 1–2 years of clinical social work experience in a healthcare or oncology setting preferred
• Experience working with medically complex or oncology patient populations strongly preferred
• Strong assessment, counseling, and crisis intervention skills
• Knowledge of community resources and healthcare systems in NYC
• Excellent communication, organizational, and interpersonal skills
• Ability to work effectively in a fast-paced, team-based environment

Preferred Qualifications

• LCSW with psychotherapy experience
• Prior experience in oncology, neurology, palliative care, or hospice settings
• Bilingual (Spanish or other languages) a plus

Skills

• Previous Social Work experience
• Proficiency with EPIC electronic medical records preferred
